Salary: £39,539 - £41,772 per annum plus excellent benefits
Location: Walsall, West Midlands (minimum 4 days in the office/community depending on business needs)
Contract: Permanent, Full time, 37 hours per week

We’re looking for an experienced and driven Social Prescribing Manager to lead impactful services and deliver successful programmes that support vulnerable customers to improve their wellbeing, sustain their tenancies and achieve better life outcomes.

Main job responsibilities:
- Lead, motivate and develop a team to deliver a high-quality, person-centred service across each programme
- Oversee programme delivery and ensure strong performance against targets
- Embed the Wellbeing Outcomes framework to evidence impact and drive continuous improvement
- Ensure effective case management processes are in place and consistently applied
- Build strong working relationships across whg to increase referrals and service visibility
- Develop partnerships with external organisations including health, social care and community services
- Use data and insights to target support to those most in need
- Oversee reporting, ensuring clear evidence of outcomes and performance
- Manage programme budgets in line with financial procedures
- Lead on safeguarding, risk management and multi-agency working
We're looking for someone who has:
- A relevant qualification or significant experience (including lived experience) of the impacts of health inequalities driven by the wider/social determinants of health
- Experience of leading and motivating a team to achieve individual targets and KPIs.
- Experience and good operational knowledge of Social Prescribing, health assessment and wellbeing outcome frameworks, tools and techniques.
- Experience of working with social housing customers and other vulnerable groups and the proven ability to intervene and to build activities and programmes to improve their long-term outcomes and overall life chances.
- Experience of working with disadvantaged groups including social housing customers, young people leaving care, homeless people, people with long term health conditions, people on a low income.
- Experience of developing, delivering and evaluating health and social inclusion focussed projects/programmes
- Experience of partnership work with a focus upon working with the community and voluntary and statutory organisations in order to increase access and take-up of services.
- Experience of managing externally grant funded programmes and reporting performance in line with the funder’s requirements.
- Experience of managing budgets in line with financial regulations.
- Experience of safeguarding and health & safety risk assessments
- A demonstrable commitment to social justice and a passion for improving outcomes for social housing customers.
